HAVE YOU FILLED OUT YOUR SCHOLAR(S) EMERGENCY CONTACT FORMS?
We cannot overemphasize the importance of this vital communications tool. Every student must have an updated family card on file, with operating phone numbers for parents/legal guardians and an emergency contact number.
If you have multiple students, complete a form per EACH student's specific grade level.

Occasionally, CMIT North Elementary may host an event in which a bounce house is scheduled to be on the premises as a fun activity for our scholars. In order for your scholar(s) to participate we ask that you fill out the attached CMIT North Elementary Bounce House Waiver Google form as soon as possible. While having the forms filled out ahead of time will allow for proper planning, notifications will go out when a bounce house is scheduled to be on the premises for an event.
We will keep the forms on file for the 2023-2024 school year. Please fill out a separate form for multiple students. In the event you need to make a change please fill out the form again.
