*Completion of 2024-2025 Dragonfly Max, an updated physical, a proof of residence, and grades verified will gain athletic clearance.
*Game Admission Fees
$5 for adults and students for the following sports: baseball, basketball, football, soccer, volleyball, & wrestling.
$2 for adults and students for regular season track & field meets
$5 for adults and students for the Tri-County Conference Meet.
Golf and Tennis are free!

Athletic Registration Information
How To Get Started With Dragonfly - Follow the directions under the "Parents & Students" column.
Dragonfly Website - Parent/guardian should register their student athlete within the Dragonfly platform. You will only need to create one account each school year. Fill out all forms electronically except for the “PPE Form” questions, NCISAA forms, and uploads.
Pre-participation Physical Evaluation Form - must have a current Pre-participation Examination (PPE) form on file at Crest Middle School in order to participate in sports. A hard copy is mandatory!
CMS Athletic Department Philosophy
The Crest Middle School athletic program attempts to provide students with many opportunities to compete in sports. Through physical participation on a team, the student is encouraged to develop favorable attitude of sportsmanship, cooperation, responsibility, loyalty and a sense of contribution to a group effort.
