Referral Process

The nature of the referral will dictate the Circle of Support member that will complete the A.C.E.S. referral. In the event of a parent, teacher, or student referral it will be assigned to the appropriate support personnel.

1. 1. DES Behavior/Re-Evaluation Data-School Counselor

   2. SST/504/Initial Evaluation Data-School Psychologist

   3. Disciple/Behavior-Behavior Intervention Specialist

   4. School Connectivity -School Social Worker

   5. Parent/Student Direct Request

   6. Referral from Safety and Wellness Checklist
