2023-2024  PARTICIPATION PACKET

Athletic Physicals and Participation Packets are due before your student can participate in a fall sport.  A physical must be completed after May 1st for athletes to be able to participate. Please do this as soon as possible. Complete the ONLINE steps after your student has completed the physical. 

AFTER you have the physical--please sit down with your student to complete the ONLINE steps together.

Overview of Online steps:

Step 1 - Read through online instructions.

Step 2--No need to print any of the documents--can be completed online.

Step 3--Parent must have/create a gmail.com account. This is where you will upload the physical form. You ONLY need to upload the side of the form that the doctor signed. Once you sign and submit the form in Step 3, parents are now done with their portion. If you are not able to upload the form you may e-mail or hand deliver the signed physical form to kmanzit@lps.org or the Athletic/Activities office.

Step 4—Use your student's chrome book - Student must sign in using their school email account (their 6 digit ID) xxxxxx@class.lps.org to sign their portion of the consent. Again, once the student has submitted their signature, they are now complete. 

When all these steps are complete your student is eligible to participate in the athletic/activity for this school year!
