One program that is greatly beneficial to students who qualify for DDA is the School to Work Program. This is a two year immersive job exploration and job development program that works with the school district and your students teacher as well as with a Job Coach from an outside community agency. The goal of the Job Foundations/ School to Work program is to assist students in obtaining a job that is a good fit for them by the time they exit school.
When applying for DDA Services the following forms are required:
1. Request for DDA Eligibility Determination (14-151)
3. Notice of Privacy Practices for Client Confidential Information (03-387)
4. Documents that support that you have a developmental disability, as described in DSHS Form 14-459 Eligible Conditions Specific to Age and Type of Evidence such as:
Educational records
Psychological records
Medical records
