🎯Click on the IHSA Medical Form 

Step 4

🎯Agree to the Electronic Signature use

🎯Fill in Student and Parent information

🎯Scroll down to the signature page and        click the signature icon

🎯Click Submit

🎯A pop-up will remind you to have your

     student sign the document in their

     student portal

🎯Close the signature window to 

     continue to registration

NOTE - you will not be able to advance in the registration without opening this form

Look for the GUARDIAN indicator, which shows up after you have completed the form

NOTE - You must complete the checkout before the student's document becomes available in their portal. The Student Signature Required indicator disappears when the student completes their portion.

NOTE - you have the option to upload the student's physical examination during this registration process.

Although this is optional during the checkout process, all students must have a current physical exam document on file to participate in sports.
