1 Original Report Card for Senior High School Graduates
Certificate of Rating (COR) for ALS Completers
Transfer Credentials with a photocopy of Transcript of Records for Transferees
Certificate of Good Moral Character
3 Loading slip for transferees
2 copies of NSO/PSA Birth Certificate
2 copies of NSO/PSA Marriage Certificate for married female applicant
2 pcs recent 2x2 ID picture
1 long white/cream folder
1 long brown envelope
1 Long size Plastic Envelope
Prepare 130php for insurance
Original Documents
- SHS Card/Transcript of Records for Transferees
- Certificate of Good Moral (GMC)
- Honorable Dismissal (For Transferees)
MEDICAL REQUIREMENTS FOR STUDENT ENROLLMENT
Academic Year 2025-2026
First Year Students:
Recent Chest X-ray (taken within the last 6 months)
Recent Medical Certificate signed by a licensed physician
Filled-out Medical Health Form
One (1) recent 1x1 inch photo
One (1) long brown envelope labeled with the student's Full Name (Last Name, First Name, Middle Name) and Academic Program
Second Year to Fourth Year Students:
Recent Chest X-ray (taken within the last 6 months)
Recent Medical Certificate signed by a licensed physician
Note: Students diagnosed with respiratory conditions, particularly Pulmonary Tuberculosis (PTB), are required to submit a Pulmonary Clearance as a prerequisite for enrollment

To be printed and submitted to the Office of the Registrar upon enrollment.
