Congratulations to all applicants who have successfully qualified for enrollment in the Bachelor of Science in Information Technology (BSIT) program of the College of Computer Studies, CHMSU Binalbagan Campus! 

Qualified applicants must confirm their slot by June 26, 2026. Failure to confirm within the prescribed period may result in the forfeiture of your slot.

⚠️ IMPORTANT REMINDERS

• Slots are strictly non-transferable. If you decide not to pursue your admission, you may not give or assign your slot to another person, including relatives or friends.

• Please ensure that all required documents are complete to facilitate a smooth enrollment process.

📑 REQUIREMENTS FOR ADMISSION

For New Entrants (First Year Students and Transferees):

• Original Senior High School Report Card

• Certificate of Rating (COR) for ALS Completers

• Transfer Credential with photocopy of Transcript of Records (for Transferees)

• Certificate of Good Moral Character

• Loading Slip (for Transferees)

• Two (2) copies of NSO/PSA Birth Certificate

• Two (2) copies of NSO/PSA Marriage Certificate (for married female applicants)

• Two (2) recent 2×2 ID pictures

• One (1) long white/cream folder

• One (1) long brown envelope

• One (1) long-size plastic envelope

• ₱130.00 for insurance

🩺 MEDICAL REQUIREMENTS

For First Year Students:

• Photocopy of a recent Chest X-ray Result (taken within the last six months)

• Photocopy of a recent Medical Certificate signed by a licensed physician

• Filled-out Medical Health Form (to be issued at the Medical Clinic)

• One (1) recent 1×1 ID picture

• One (1) long brown envelope labeled with the student’s Full Name (Last Name, First Name, Middle Name) and Academic Program

Note: Students diagnosed with Pulmonary Tuberculosis (PTB) must present either:

• A certificate stating that they have completed at least 14 days of treatment and are no longer contagious and are fit to attend school; or

• A Certificate of Treatment Completion issued by the TB-DOTS Program of the City Health Office or their attending pulmonologist.

The College of Computer Studies is pleased to release the last and final set of applicants qualified for the Final Interview (Batches 3 & 4) for the Bachelor of Science in Information Technology (BSIT) Screening and Evaluation Process at CHMSU Binalbagan Campus.

🗓️ Final Interview Schedule:

• Batch 3: June 22, 2026

• Batch 4: June 23, 2026

📌 Please bring the following on your scheduled interview date:

• A valid ID with a recent photograph

• Physical copy/copies of your Grade 12 Report Card(s)

• Physical copies of relevant certificates (academic, leadership, extracurricular, or skills-related)

Note: Additional batches for the Final Interview will be announced next week and are scheduled to take place on June 22, 23, and 24, 2026. Applicants who are not included in the first two batches are advised to stay tuned for further announcements.

• Venue: CHMSU Binalbagan Campus (Check your specific room assignment in the list)

• Arrive Early: Please be at your assigned testing room at least 30 minutes before your scheduled time. Latecomers will not be entertained.
• What to Bring: 
  • Valid ID with pictures (School ID or Government-issued ID)
  • In a brown envelope: Copy of Grade 12 Report Card or DepEd Form 138. If transferee or shiftee: Transcript of Records (TOR)
  • Black ballpen and Pencil (No. 2)
• Dress Code: Wear decent and proper attire. Please strictly adhere to the campus dress code policy (no slippers, shorts, or sleeveless shirts).
