Each instructor Summer Camp Instructor will have a CARD binder with all the physical forms you will need on-site. Please request new forms as needed. The content of the binders will include:
Program Rosters
Sign in & out sheets
CARD Contact Sheet
Emergency Contact Forms
Accident/Incident Forms
Ride/Walk home form
Rosters are used to verify that participants are properly registered through CARD.
Sign-in and sign-out sheets must be used daily at both drop-off and pick-up.
The CARD Contact Sheet includes key contact information, such as the Supervisor, Coordinator, and Main Office.
Instructors must ensure that parents or guardians turn in or complete the Emergency Contact Form on the first day of the program.
Accident Reports must be submitted to the CARD office within 24 hours of the incident.
Parents may authorize their child to walk or ride home after camp by signing a permission form; however, all campers must be signed in by a parent or guardian each morning.
