COVID-19 Monitoring Screeners
Student COVID-19 Monitoring Screener
Each student or family member acting on behalf of their child must complete a screener prior to entering a district building. Without a completed screener, students will not be permitted to participate in in-person instruction. Please be advised of the following guidelines:
The screener MUST be completed every day no more than two hours prior to reporting to any district building. One form per student. Per HIPAA, all information will be kept confidential.
For ease of access, every morning students are in school, families and 7-12 grade students will receive email and text (provided their number is in the system) reminders containing the webform link. | Text message registration> | Update contact information>
The webform can be accessed via cheltenham.org/COVID19StudentScreener or via a scannable QR code posted at the entrance ways of every school.
Complete the contact information screen and indicate to what building the student is reporting.
On the next screen, answer all of the COVID-19-related all questions completely.
After completing the screener, you will receive either a “yes” or “no.”
Staff/Visitor COVID-19 Monitoring Screener
The form can be accessed via cheltenham.org/COVID19MonitoringScreener.
This screener MUST be completed every day no more than two hours prior to reporting to any district building.
Once logged into the system with their email, staff must complete all of the questions provided and submit the form.
All information will be kept confidential.
After completing the screener, you will receive either a “yes” or “no.”
“Yes” means you are cleared to enter the building.
“No” means you are not cleared to enter the building, and will be receiving a call from the nurse in a few minutes to confirm your building access status.
