Volunteer Protocol and BCI Check Forms

The following information is provided to all volunteers. It is especially important to those who wish to chaperone field trips and to those who wish to volunteer in classrooms. Please read, sign, and return the Volunteer Protocol and BCI check forms to the Chariho Administration Building located on the Chariho High School campus.

General Information

1. Volunteers must agree to an annual background check.
2. Volunteers must be at least twenty-one (21) years of age.
3. Volunteers must sign in and out of the school office and wear a visitor's identification badge.
4. Volunteers must give as much notice as possible when cancellation is necessary.

The background screening needs to be completed and submitted to Chariho with a CLEAR picture of the front and back of your license.
The clerk at the Chariho administration building can notarize your form for FREE! Or you can have the form notarize at your local police station.
BCI needs to be completed annually for anyone who wants to volunteer for any club or program.
