Each student-athlete must have an account on Dragonflymax.com for VALLEY HIGH SCHOOL (C9KYVV).

All athlete and parental forms must be signed and uploaded to DragonFlymax.com for each student-athlete before participation.

FORMS INCLUDED: AN UPDATED MEDICAL, DEMOGRAPHIC AND PHYSICAL FORM, NFHS SPORTSMANSHIP, CONCUSSION INFO, VHS CONSENT FORM, CHAMBERS COUNTY DRUG FREE FORM, PARTCIPATION AGREEMENT AND BIRTH CERTIFICATE.
