Immunizations & Medical Requirements

Pre-Kindergarten

1. Physical on Illinois Department of Human Services form, signed by a physician, NP, or PA and the following immunization by October 15th:

Varicella – 2 doses

MMR – 1 dose

Hep B – 3 doses

Polio – 3 doses by 2 years old

DTap – 3 doses by 2 years old

HIB – 4 doses

Pneumonia – 4 doses

2. Lead Assessment – due by the first day of school

3. Dental examination by May 15^th (our district dos these through SIU)

Kindergarten

1. Physical on IDHS form and the following immunizations are due by October 15^th. THE ONLY EXCEPTION TO THIS IS HOMELESS STATUS! NO APPOINTMENTS WILL BE ACCEPTED.

Varicella – 2 doses

MMR – 2 doses

Hep B – 3 doses

Polio – 4th dose must be after 4^th birthday

DTap _ 4^th dose must be after 4^th birthday

2. Dental examination by May 15^th (our district does these through SIU)

3. Vision exam by eye doctor (school physical does not count)

2^nd Grade

1. Same as kindergarten

6^th Grade

1. Same as kindergarten as well as DTap booster and MCV4 (meningitis)
