Required Forms/Fees

Please complete the following required forms/fees:

1. Field Trip Parent Permission - Due Aug. 15

   • IMPORTANT: Parents/guardians provide permission for their child to attend CSD field trips online in Skyward Family Access as part of the online registration process. Online registration closes Aug. 15.

2. YMCA Camp Minikani Release/Waiver - Due Aug. 15

   • IMPORTANT: Parents/guardians provide their signature for the YMCA Camp Minikani Waiver in Skyward Family Access as part of the online registration process. Online registration closes Aug. 15.

3. Camp Minikani Field Trip Fee ($70) - Due Sept. 6

   • The field trip fee will be assigned to your child in Skyward Family Access and will need to be paid online through Skyward (Fee Management section). 

   • If the cost creates a difficulty for any student or family, please contact the  Webster office.

4. Medication Authorization 

   • If your child needs prescription medication or requires an Epi-pen or inhaler OUTSIDE OF THE SCHOOL HOURS (7:30-2:47 PM), a Medication Authorization is required. Please email it to ajohnston@cedarburg.k12.wi.us or drop it off in the main office. ALL MEDICATION NEEDS TO BE TURNED IN TO THE WMS HEALTH ROOM BY WEDNESDAY, SEPTEMBER 18. 

   • Any medication regularly taken at Webster where parents have already filled out a Medication Authorization form (Rx, inhaler, epi-pen) will be sent to Camp Minikani and administered as directed. NO additional forms are required.
