BMI Survey

Parental Notice for 2021-22 School Year

As part of a required school health examination, a student is weighed, and his/her height is measured. These numbers are used to figure out the student’s body mass index or ‘BMI’. The BMI helps the doctor or nurse know if the student’s weight is in a healthy range or is too high or too low. Recent changes to the New York State Education Law require that BMI and weight status group be included as part of the student’s school health examination. A sample of school districts will be selected to take part in a survey by the New York State Department of Health. If our school is selected to be part of the survey, we will be reporting to New York State Department of Health information about our students’ weight status groups. Only summary information is sent. No names and no information about individual students are sent. However, you may choose to have your child’s information excluded from this survey report. This survey is for grades pre-K, Kindergarten, first, third, fifth, seventh, ninth and 11th grades.

The information sent to the New York State Department of Health will help health officials develop programs that make it easier for children to be healthier.

If you do not wish to have your child’s weight status group information included as part of the Health Department’s survey this year, please print and sign your name below and return this form to:

NYS requires Vision and Hearing screenings for new students, and students in kindergarten, 1st, 3rd, 5th, 7th and 11th grades. If students in the grades listed above do not have a current vision and/or hearing screening on file then screenings for those students will be done at school.

Staying Healthy Tips

Please review these important tips for helping to reduce the spread of germs:


1. Take 5 (minutes) to Disinfect

  • Phones

  • Doorknobs

  • Refrigerator handles

  • TV remotes

  • Toy storage bins

  • Light switches

  • Faucets

2. Teach Kids the ABCs of Healthy Hygiene

  • Always wash hands before and after eating and after using the bathroom, with soap and warm water, for at least 20 seconds — the time it takes to sing "Happy Birthday" twice.

  • Be aware of germs, and know where they can hide, especially on shared school supplies.

  • Cough and sneeze into elbows or tissues, not hands. And don’t forget to have the students wash or hand sanitize their hands after!!!

Nurse's Notes

EXCUSES

We must have a written excuse for all absences or lateness, regardless of the reason. Please write an excuse upon your child’s return to school. All students coming in late must check in with Mrs. Snyder's to obtain a pink pass prior to going to their classroom.

BUS CHANGES

Please send in a note for any bus change. If we do not have a note, your child will be placed on his/her regular bus and get off at his/her normal stop. If you need to make an emergency change, please do so by 1:00 p.m. Telephone calls, which cause many interruptions to classes, are strongly discouraged.

EARLY DISMISSAL (Doctor appointments, dentist appointments, etc.)

Please send in a note the day of the appointment and the time you wish to pick up your child. This makes staff and teachers aware they are leaving, and students will be ready to go. For last minute emergency changes, go to Mrs. Snyder's office to sign your child out.


MEDICATIONS

For medications to be administered in school, they must come labeled in a pharmacy bottle with a note from the doctor stating it must be given during the school day, directions for administration and also a parent note requesting the school nurse to administer it. This also includes all over-the-counter medicines. Do not send full prescriptions (just enough for the duration). Medications must be brought in by a parent or a parent designee. Students are not allowed to carry medications on their person unless the proper paper work for self-directed/able to carry is filled out by a Doctor and is on file in the Nurse's office.

****Per NYS Law and Regulations- Self-directed students are only allowed to carry inhalers and epi-pens. Even with a prescription from a doctor and parent permission, students should keep ALL other medicine in the nurse's office.

REQUIRED GRADE PHYSICALS

NYS & District policy requires school physicals for all new students as well as students in Kindergarten, grades 1, 3, 5, 7, 9 and 11. You may take your children to your own doctor, if desired. If you choose to do this, please notify the nurse at 688-4472, and she will send home the physical form or click HERE to download the form. There is no charge for physicals done at school.
