Donations Forms

1. Using the electronic copy of the donation form, type in your name and address so that donors know to send the donation to you. It also tells us who the donation is for when processing your donations.
2. Ensure the donor’s address is filled in completely; otherwise, they will not receive their tax receipt. Phone number and email are not essential, but helpful if we need to reach them if there are any problems with the donation.
3. If there is a corresponding cheque, staple it on top of the donation form to the top left corner.
