Purpose: This survey is part of an academic research project studying discipleship practices in Christian Church (Disciples of Christ) congregations.
Who should respond: A pastor, minister, or senior lay leader familiar with your congregation's discipleship programs.
Time: Approximately 8 minutes.
Confidentiality: Individual congregations will not be identified in published findings without explicit permission. Data is stored securely and used solely for this research.
Voluntary participation: You may stop at any time. There is no penalty for withdrawing.
