Oakstone Academy Pizza Program:

Open to students in grades K-12 and staff members at Oakstone Academy. Please fill out a form for each person ordering pizza and follow all instructions.

Pizza Program Calendar

This calendar shows all order collection windows, deadlines, and pizza Fridays. If you would like to order pizza for your child, use the forms below during the designated order window. Please read carefully and follow all directions.

Pizza Order Forms

Pizza order forms are open only during the ordering windows. Please ensure you are using the form for the building you student eats lunch in. (Elementary age students in Ms. Storie's class and Pathways participants should use the 939 form.)

Grades K-5

Elementary students should turn their money in to Ms. Jen or their classroom teacher.

Grades 6-12

MSHS students should place their money in the pizza money box near the front desk.

* Staff, please use the form for the building you would like it delivered to. Building 900 staff, please use 939.
