Massachusetts law (Chapter 76, Section 15) states that no child will be admitted to school unless they show proof that all the vaccines required by the Department of Public Health have been received. The CBRSD Schools will exclude any child from school entry until the school nurse receives this written proof. If parents/guardians need help in this area they are encouraged to contact the school nurse.
There are three situations in which children not appropriately immunized may be admitted to
school:
A medical exemption is allowed if a physician certifies that immunization is medically contraindicated. This must be renewed annually at the start of the school year with health services.
A religious exemption is allowed if a parent or guardian provides a written statement that immunizations conflict with their sincere religious beliefs. This should be renewed annually at the start of the school year with health services.
Homeless children and children in foster care are protected from exclusion by the McKinney-Vento Act.

Vaccinations: Hib: 1-4 doses DTaP: 4 doses Polio: 3 doses Hep. B: 3 doses MMR: 1 dose Varicella: 1 dose

Grade Level Requirements
A copy of the report of a physical examination and a copy of the required immunizations are also required for all children entering into grades 4, 7 and 10
