Outside of School Community Service Form

To complete community service outside of our school projects, you will need to 

1- get prior approval from Ms. Wooten, Mr. Moore  or Mr. Ogle 

 2- after the event, submit the below form complete with the supervisor's signature. If you have any questions, see a sponsor.
