College Day Form

Use this document when requesting a college day. Once official proof is turned into the counseling office your attendance will be fixed. Students are allowed 3 days their junior and senior year.

This can be used for:

• Official campus visits
• Scholarship interviews
• College placement tests
• Enlistment meetings
• Job interview for a full time job after graduation
