New Protocol Approval Request - What's required?
Protocol Approval Request: Project approval is obtained by submitting a protocol proposal to the BSL-3 RA who will take the request to the BSL-3 Director and Advisory Group.
New protocol requests will be presented at the next scheduled Advisory Group meeting for approval. Meetings are scheduled quarterly or as needed in the case of an emergency.
New protocols will be approved when all conditions put forth by the committee are met and addressed within the protocol. Additional meeting may be required.
Any and all changes to approved protocols must be submitted the BSL-3 RA for approval by the BSL-3 Director.
Protocols will be reviewed annually.

Removing items from the BL3: Please complete and submit this form PRIOR TO removing your item from the BL3! Items to be removed from the BL3 must be approved and follow a strict protocol for decontamination. Failure to report or follow the procedures for decontamination and removal may result in removal of your BL3 privileges.

Donning and Doffing your PAPR
Headpiece assembly - you definitely need this. You may order these through smart cart. Each person will need 1, however I suggest ordering at least one extra (a few if there are multiple researchers per lab). Item no: 19-134-241.
