Work Permit Info

Work Permit Application Instructions: 

1. Download the work permit application form

2. The work permit application form must have the following sections completed before being processed: student  information, parent/guardian signature, and employer information. 

3. Bring the completed application form to Ms. Hannah Yen for next steps during the school year. Ms. Yen's office is located in the Sage Creek Counseling Center (Registrar, Room 3004).

4. Summer Work Permits will be processed June 20th -August 2nd at the CUSD District Office, 6225 El Camino Real Carlsbad CA 92009. Please email studentrecords@carlsbadusd.net or call 760-331-5075. Please note: the turnaround time will be 24-48 hours to process.
