If you have a damaged school device that needs repair, or will need to be swapped, please submit a ticket with the Technology Department CPA Ticket Form.
Provide a brief description of the issue you are having. Include the student's first initial and last name, district ID, and the asset tag of your student's device, which can be found on the back of your students technology. If the device is damaged please provide a description of how it happened. *Restitution charges may be applied for damage due to negligence.
Once you submit a ticket, if the issue cannot be resolved or if there is hardware damage, your student will receive a replacement device, which will be swapped out for the student's damaged device. The student's replacement device will be assigned to your student for the remainder of the school year or for as long as they are enrolled with the school.
