WVSA Partner Information form:

There will remain ONE partnership information form.

Our districts will continue to use the partner info form to share contact information and documents that are necessary for partnering.  

Please only complete this form once.  

After you have uploaded the necessary documents to the form, districts will connect around individual contracts.

When you complete the partner information form please have accessible the following documents to upload:

1. STARS certificate

2. Certificate of general liability insurance

3. Certificate of worker's compensation insurance (if separate)

4. Tuition statement

5. Attendance policy

6. W9

7. Program Calendar for school year 26-27

8. Pre-qualified assurance certificate from AOE

9. Teacher’s license

10. Program family handbook
