Indicator 13

Percent of youth with IEPs aged 16 and above with an IEP that includes appropriate measurable postsecondary goals that are annually updated and based upon an age appropriate transition assessment, transition services, including courses of study, that will reasonably enable the student to meet those postsecondary goals, and annual IEP goals related to the student’s transition services needs. There also must be evidence that the student was invited to the IEP Team meeting where transition services are to be discussed and evidence that, if appropriate, a representative of any participating agency was invited to the IEP Team meeting with the prior consent of the parent or student who has reached the age of majority. (20 U.S.C. 1416(a)(3)(B))

#1    -    MEASURABLE POSTSECONDARY GOALS

               a) Can the goals be counted?

               b) Goals occur after student graduate?

               c) Goals are appropriate for student

               d) REQUIRED GOALS:   1.   Education and Training     2.  Employment

                e)  Need Independent Living goals, as needed

                IMPORTANT:    Strengths, preferences, and interests  should drive and support postsecondary goals

 #2      -     POSTSECONDARY GOALS ARE UPDATED ANNUALLY

                a) Appropriate goals are based on DATA e.g.  information about the  student's strengths, preferences,   and interests

#3       -      TRANSITION ASSESSMENTS ARE AGE APPROPRIATE

                 a) Measures strengths, preferences, and interests

                 b) Require "Parent Permission" if the instruments are specific to student not the whole population

                 c) List the names of assessments and the results

#4        -       TRANSITION SERVICES AND ACTIVITIES

                   a) More activities should be directed to Measurable Postsecondary Goals 

                   b) Provides DATA for strengths, preferences, and interests

#5         -        COURSES OF STUDY

               a) Should have MORE THAN ONE course to support postsecondary goals

               b) Should have a CLASS that support postsecondary goals (Obvious choice is Better!)

               c) Course description should be multi-year 

               

            #6         -     ANNUAL IEP GOALS

                           a) At least ONE IEP GOAL tied to Education/Training and Employment

                           b) Goals should be skill specific

#7           -      PARENTS AND STUDENT ARE INVITED

                      Important:  Show evidence that parents and student are invited to the meeting!

#8           -       OUTSIDE AGENCY REPRESENTATIVES ARE INVITED

                      a)  With Prior Consent from parent and/or student

                      b) Show evidence that parents and student are invited

ADDITIONAL INFORMATION

 1)  INCLUDE statement that the child has been informed of the child's right (Procedural Safeguard): 16 years old before child turns 17.
