Guest Form & Appeal Form

Guests must be approved in order to attend prom.  Turn these in at the front desk (you can get printed copies there too); a background check will be conducted.  Guests must be not be over the age of 20.  Guests must be, at minimum, a freshmen in high school (any school is fine).

If your guest does not attend a high school/college, their employer must fill out the form.

Guest forms must be turned in by Monday, March 30th, 2026.  There are NO exceptions.  If a guest form is not turned in, your guest will not be attending prom.

If students have any OSS or more than 3 days/12 periods of ISS they are ineligible to attend prom, but they may submit an appeal to the administration.  Appeals are accepted from March 9th through April 17th. Students will be notified of the appeal decision within two weeks of the appeal’s submission. Freshman, sophomores, and guests are not able to appeal.
