Food Allergy Action Plan (American Academy of Pediatricians - AAP) to be completed by parent and physician in regards to specific information on the treatment of a child with a food allergy in the event of a possible or certain ingest of offending food. You only need to complete either this form or the one below from FARE.

Prescription Medication Form to be completed by physician and parent for emergency medications of Epinephrine, inhalers and/or antihistamines such as Benadryl.

Over-the-Counter (OTC) Medication Form to be completed by parents/guardians. The OTC medications listed on this form are only intended to be given three times or so during the school year.
