2. Electronic copy of Certified Photocopy of SF-10 (Junior High) and SF-9/Report of Card (G11) or Certification of Grades from the School Principal)

3. Electronic copy of Certificate of Good Moral Character with documentary stamp (PDF Format)

4. Electronic copy of the passport-size picture on white background with nametag: 

First Name, Middle Initial, and Last Name (as indicated in your PSA/NSO) (jpg format)

5. Electronic copy of PSA/NSO Copy of Birth Certificate (PDF Format)

4. Transcript of Records/Scholastic Records and Diploma or Certificate of Completion (with English Translation) duly notarized and authenticate by the Philippine Embassy or Consulate

5. Notarized Affidavit of Support and proof of adequate financial support

6. Photocopy of passport bio-page

7. Birth certificate or its equivalent duly authenticated by the Philippine Foreign Service Post (PFSP)
