Be Government issued:
National (e.g. passport, military ID, green card)
OR
State (e.g. driver's license, ID card, weapons permit, Inmate ID card)
Have a photo - the photo does not have to be crystal clear
Clearly show ALL the following information:
ID number
Date of Birth
Expiration Date (the ID cannot be expired)
Special notes:
You may photocopy and mail, or take a picture and email or text
The address on your Identification does NOT have to be your current address
Please refrain from faxing IDs as faxing may make the information illegible
If submitting a passport, please ensure that ALL the numbers on the passport are visible

Voter registration
SSN card
Employee ID card

The Notarized Sworn Statement must be completely filled out and signed by you in front of a Notary.
The Notary must NOT write "personally known", and must indicate the "type" of ID used (e.g. FL DL, Passport, Inmate ID).
Most banks perform notarization services. The UPS store also provides notarization services (in Florida the fee can be $10).
For claimants that are unable to apply to their DMV for a current ID (e.g. persons who are infirm or disabled), a form called an Affidavit Attesting to Claimant's Identity may be used. You can download that form HERE.
