A Graduate Assistantship (GA) award covers full payment of the monthly premium costs for the GA health insurance plan. The current GA plan is a student plan and the insurance provider is Aetna using the Open Choice PPO network.
If you are a current student, visit the Aetna Member Portal as your one-stop for all your plan information needs. The portal includes materials for the GA health insurance plan, how-to videos, benefits summaries, pharmacy information, and forms.
You can also access your Aetna Student Health Insurance ID Card through the portal using your Student ID, Date of Birth, and Student Email (name@u.boisestate.edu). If you experience any issues accessing it, please contact gradassistant@boisestate.edu for assistance.

When seeking medical care, always confirm that your provider is in-network and understand your plan’s benefits before your visit.
The Graduate College handles your insurance enrollment, but any specific questions about benefits, claims, or billing should be directed to Aetna.
Taking a few minutes to familiarize yourself with your plan will help you use your coverage confidently and avoid surprises.
GA health insurance coverage starts on the first day of the month of your first day of work (i.e., if the first day worked is August 1-31, insurance begins August 1). GA health insurance coverage ends on the last day of the month of your last day of work (i.e., if the last day worked is May 1-31, insurance ends May 31).*
*Exception: If you are a GA in the Spring term and receive a confirmed appointment for the following Fall term by June 1, your GA insurance will continue during the summer without a break in service.
Plan year dates as it relates to the annual deductible and out-of-pocket limits are August 1 – July 31.
Enrollment in the GA health insurance plan is automatic and mandatory. GA health insurance can be waived if you have other eligible coverage. Contact gradassistant@boisestate.edu directly to request a health insurance waiver form.
The GA dental benefit is separate from GA health insurance. This dental benefit is a preventative benefit only, and includes the following:
Visits & Exams:
Oral evaluations, (2 routine visits and 2 problem-focused visits per year)
Prophylaxis (cleaning) or scaling-moderate/severe inflammation–full mouth or periodontal maintenance, (2 treatments per year)
Images and pathology
Bitewing images (1 set per year)
Entire dental series, including bitewings or panoramic film (1 set every 3 years)
Vertical bitewing images (1 set every 3 years)
Periapical images
GAs may purchase dental and vision insurance separately through Aetna.
University Health Services provides on-campus primary care, urgent care, counseling, and immunizations and offers additional care that may be included in your health plan such as visits with a dietitian and massage therapy. Most services are provided under the GA plan at no charge. The GA insurance deductible is waived at UHS.
After your GA position has ended, you have the option to purchase up to 3 additional months of coverage on the GA health insurance plan. You must pay the monthly premium(s) directly to Aetna. Questions? Contact Aetna Student Health at 1-877-480-4161 (TTY: 711).
To request proof of coverage or a loss of continuation form, please contact Aetna Customer Service directly at 1-877-480-4161. You may also request documentation through the Aetna Member Portal. Once registered for the portal, select Message Center, type in your request, and send.
