New Student Enrollment

To enroll your student at Borah High School, please contact the Counseling secretary, Nicole Ouellette:

1. Enrollment packet

• • • PAPER: Come into the Counseling office, full packet at the bottom of the page, or have it emailed to you

    • ONLINE: click on online registration system

2. These documents need to be attached to the online registration system OR turned into the Counseling office.

   • Home Language Survey 

   • Verification of Residency form

   • Parental Consent for Health Care form

   • BSD Health History form

   • Idaho Migrant Education Survey form

3. Proof of Residency 

   • Any utility bill, lease agreement, or mortgage bill

   • If you are living with a family member or a friend, they will need to provide a note stating that they are aware that student and guardian(s) lives with them, as well as a copy of their proof of residency.

6. Transcript

7. Copy of IEP/504 (if applicable)

* If you would like to open enroll at Borah, you will need to fill out an open enrollment application. 

• OPEN ENROLLMENT APPLICATION IS NOW ONLINE ONLY

• It will then get approved or denied by the Principal. If approved, we can move forward with the documents needed for enrollment. If it is denied, you will need to attend your home school or contact the District to set up an appeal appointment. 

How to Withdraw a Student

If you need to transfer your student to another school, whether inside or outside the Boise School District:

• Complete the Withdrawal Form (fillable PDF), then submit it to the Borah Registrar via email or you can drop it off.
