When a child with developmental disabilities reaches the age of 14 years old or the 8th grade, the Transition Planning will begin. Transition planning and services are initiated by the school and documented in the student’s Individualized Education Program (IEP). Each school has a Transition Team on site for the students. As a team, we help students with Interest Inventories, Job Applications, Resumes, Self Advocacy, Work-Based learning opportunities, and Post-Secondary Opportunities. The Transition Team will assist the families in planning for the future whether it be college, employment, or even a living facility.
