Reminder for Current 5th Grade Parents
Please be advised that in accordance with N.J.S.A. 18A:40-21.2, the New Jersey Department of Health and Senior Services requires the following two vaccines for all students entering 6th grade in September 2023 (given @ their 11 yr old checkup):
Meningococcal vaccine, one dose
Tdap vaccine (booster), one dose
Students will not be allowed to enter 6th grade without a Health Care Provider’s documented proof of these vaccines.
Please submit this to me ryoutkus@bernardsboe.com no later than June 9, 2023.
