Physical Exams
Regular physical examinations are important to the growth and development of your child. We request documentation of physical exams as explained below.
upon entry into the school district;
exam may have been within the
previous 365 days
- prior to participation on any school team; exam must be documented on New Jersey Department of Education Pre-Participation Physical Evaluation Form and be within 365 days of the first practice session
- pre-adolescent developmental stage-grade 4
- adolescent developmental stage – grade 7

The school physician, Dr. S.Kairys , is
available for exams if your child does
not have his/her own medical provider.
