1. Name of Solo + Composer's Name

2. Name of Accompanist (Please put NA if playing an unaccompanied solo)

3. Biography

4. Letter of Recommendation (Band Director)

5. High School Transcript (GPA, grades, etc.)

6. Letter of Recommendation (Guidance Counselor, Administrator, etc.)

7. Professional Photo/Headshot (i.e. senior photo or other professional photo)
