Avoca District 37 is requesting that all vaccinated individuals (staff and students) upload documentation of their vaccination record. Having vaccination information in advance will allow the District to more efficiently implement public health guidance in the 2021-2022 school year. For example, if IDPH guidance allows for vaccinated individuals to go to school without wearing masks, vaccination information would confirm for the school who is eligible to go without a mask. Additionally, Illinois Department of Public Health guidance has previously indicated that fully vaccinated individuals are not required to quarantine after exposure to a positive COVID-19 case, as long as the individual remains asymptomatic, so vaccination information plays a role in contact tracing and determining quarantine status.
You are not required to provide the District with documentation of your vaccination record. Please be aware, however, that those without a record of vaccination on file will be required to follow health protocols for unvaccinated individuals. All submitted records will be secure and retained in a manner consistent with the Health Insurance Portability and Accountability Act (HIPAA).

Vax Booster Clinic on January 27
If you know of someone 12+ who needs a vaccination booster, please see the details below (if you are not already signed up) for the clinic tomorrow at Misner Auditorium in Glencoe. This is run by Jewel/Osco at Misner Auditorium. You can register for the Vaccination Clinic below. Make sure to also complete the necessary forms.
Please arrive with the following ready:
A completed patient consent form
A copy of your insurance card and ID on one page
Your CDC vaccination record card
Testing Clinic on January 29
And there is another testing clinic run by North Shore Clinical Labs on Saturday, January 29 at Marie Murphy from 9 a.m. to 2 p.m. (Test results were returned more quickly after the January 22 testing clinic.)

Again, vaccinations and regular testing are among the most effective mitigation steps we can take to keep COVID rates in our schools low enough for us to operate in a healthy and effective manner.
The 16+ booster clinic at New Trier has been expanded to a 12+ booster clinic. I am not sure how many spots are left, but attached is all of the information and the link to register. We will monitor registrations and look to work with Jewel/Osco to add another clinic as soon as possible if needed.
COVID Vaccination Clinic 2 - Nov 23 & Dec 14

Registration is conducted by Albertsons/Jewel-Osco and the Administration has no control over the registration system.
(If you are registered for the first vaccination clinic, the second dose will be provided on December 3rd at Washburne School in Winnetka.)

The attached letter outlines all of the details related to the Vaccination Clinic on Nov. 12 (dose 1) and December 3 (dose 2) - taking place at Washburne School on Hibbard in Winnetka.

The registration is operated by Albertsons, the parent company of Jewel-Osco, which is operating the clinic. Please make sure you follow all of the directions in the letter regarding documentation, scheduling, parking, and more so that your child's vaccination goes smoothly.
Another clinic will be held on November 23 (dose 1) and December 14 (dose 2). The registration information for that clinic will be sent the week prior to November 23.

Planned Vaccination Clinic for Students Age 5-11 - Preliminary Notice
Please see the attached letter issued jointly by all of the New Trier Township elementary school superintendents regarding a proposed vaccination clinic for students ages 5-11 to be held in November and December. This letter is preliminary. Actual registration details will be shared on November 8. All medical questions should be directed to your pediatrician.
