File a Collaboration Request Form
Dr. Spates will review and approve accordingly by checking the Approved Box on the Collaboration Hours form.
Check the Collaboration Hours form to see if your request has been approved.
If approved, conduct your collaboration, using a meeting sign-in sheet, maintaining an agenda/minutes, and if available collecting documents created during the collab.
Log into the Collab Evidence form. Submit the meeting sign-in sheet, agenda/minutes, and if available any documents created during collab.
Payroll hours hours will be taken from the sign-in sheet. Mrs. Forbes will document the work product for potential future Title 1 audits.
