Medication Order Forms

When it is essential for your child to receive medication at school, it may be given if the following procedures are met. The parent should:

  1. Have the correct medication order forms completed and signed by the parent and doctor.

  2. Bring the medication to school in a container labeled by the pharmacy with the following information:

    • Child’s Name

    • Name of medication

    • Doctor’s name

    • Doctor’s phone number

    • Expiration date of medication

It is important that the parent have a container at home with the correct dosage and a properly labelled separate container for school. Whenever possible, we encourage you to work out the dosage schedule with the doctor so that medication is not given during school hours.

School Entry Health Assessments:

  • School Entry Physical Exam Requirement: All incoming first grade students are required to have had a well-child health exam within 18 months prior to or 90 days after entry into first grade.

  • Oral Health Assessment: AB 1433 requires that children entering public school for the first time (at either kindergarten or first grade) have an oral health assessment performed by a California licensed dentist or registered dental hygienist within 12 months prior to or by May 31 of the child's first year in public school.

  • Personal Belief Exemptions (PBEs) are no longer accepted by the state of California.

  • If your child is unimmunized due to medical reasons, please notify the Health Office staff for further guidance.

The completion of this form gives your permission to the school nurse and lead nurse to access your child's protected health information.
