You will be asked to enter an email address, so that a copy of the choices will be emailed to you when you submt the form.

Please remember that subjects are NOT allocated on a first come, first served basis. Your child has until 11th of March to complete the Options Choices Form. Miss Page will work on the timetable duing the Easter holiday.

Please complete this form with your child , you will receive an email confirming their selection to the email address entered at the top of the form. Please keep this email in case you need it for future reference.
