MANDATORY PAPERWORK
Each school year, ALL APS student-athletes must turn in the required paperwork:
NMAA Sports Physical (requires a doctor/physician visit)
NMAA Consent to Treat Form (page 7)
NMAA Concussions Form (pages 8-9)

One packet of the above forms will cover your student for the entire school year / all APS sports offerings.
A new physical packet must be submitted each school year dated after April 1 from the previous school year.
The mandatory paperwork must be filled out and turned in to the EMS Athletic Director BEFORE tryouts:
PLEASE!!! Double-check your paperwork to ensure that you have completed it correctly and completely. All parts must be filled in and all initials/signatures are required.
If you submitted an athletic packet to a high school, you still need to provide a copy of the packet to EMS (applicable to some 8th-grade parents)
Printed packets need to be turned in to the athletic director BEFORE tryouts.
If athletic paperwork is not fully completed and turned in BEFORE tryouts, your student will NOT be allowed to tryout; absolutely no exceptions.
NMAA INTERSCHOLASTIC ELIGIBILITY REQUIREMENTS
Academic Eligibility: A student shall have a 2.0 grade point average with no F’s, based on a 4.0 grading scale, or its equivalent, for the semester grading period immediately preceding participation.
For girls volleyball, previous spring semester grades will be used.
For boys basketball, fall quarter one grades will determine eligibility.
For girls basketball, fall semester grades will be used.
For boys and girls track & field, fall semester grades will be used.
For students not eligible at the semester, the nine week grading period can be used to regain eligibility if it applies.
Competing during the first semester of the school year without reference to the previous semester grading period applies only to 6th grade students. Academic eligibility requirements shall apply to all 6th grade students upon the end of the first grading period of the school year (Quarter 1).
Middle school programs directed and supported by the APS Athletic Department are recognized as New Mexico Activities Association (NMAA) sanctioned activities. Eligibility and participation guidelines set forth by the NMAA and the APS Athletic Department apply to all participating students and coaches. NMAA academic eligibility requirements are required of all participating students.

To promote the development of self-discipline and high academic/athletic expectations.
To encourage student/athletes to conduct themselves in a manner that brings respect to themselves and Eisenhower Middle School.
COMPETING SCHOOLS
Eisenhower Middle School girls volleyball, girls and boys basketball, and girls and boys track & field will compete with the following schools:
Desert Ridge, G. I. Sanchez, James Monroe, Jefferson, Lydon B. Johnson (LBJ), Madison, Tony Hillerman, & Truman.
APS CODE OF CONDUCT
Please be advised that Albuquerque Public Schools has a code of expectations for all players and spectators participating in athletics.
We appreciate your cooperation in adhering to the following obligations as a spectator at athletic events at home and away games. Our goal is to provide a high-quality experience for every person participating and to ensure the best possible environment for all student-athletes, coaches, officials, and other spectators.
EISENHOWER MIDDLE SCHOOL ATHLETE CODE OF CONDUCT
Grades: Per NMAA, all student-athletes shall maintain a 2.0 GPA with no F’s at all times during the season. Grade/behavior checks will be given mid-season. Failure to maintain grades will result in temporary suspension from the team until grades are brought up. During this time student-athletes are still required to attend practices and games to support their teammates. Failure to turn in required grade/behavior checks on time will result in sitting out for games and practices until proper form is turned in.
Behavior: As student-athletes, you represent not only yourself but also your team, your coaches, your school, and your community. Student-athletes are held to a high standard. Please review below for detailed behavior and actions.
