Consent to Participate in Research

Information to Consider About this Research

I agree to participate as an interviewee in this research project, which concerns faculty employment rights and hearing procedures. The interview will take place by Zoom videoconference. You are one of a subset of approximately 100 interviewees determined by self-identification OR researcher awareness of your experience with hearing processes.

In general, this interview should take no more than 30 minutes to an hour of your time.

• I understand the interview will be about my experiences with hearing processes related to faculty employment issues.
• I understand that no questions will ask me to violate standards of confidentiality required with regard to personnel issues and actions.
• I understand that this study may be used to make improvements in how such hearings are conducted throughout the UNC system.
• I understand that the interview(s) will be audio and/or video recorded and may be published.
• I understand that the audio and/or video recordings of my interview will be kept on a Google Team Drive site associated with this research and excerpts of my interview may be used in public presentations and publications of this research if I sign the authorization below. ALL use of information/quotations from interviews will be by ROLE and not by NAME (i.e., “a member of a hearing committee said,....” or “ a provost observed that......”). No video recording itself will be used in public presentation OR will be published.

I give Dr. Anderson and Dr. Gravett ownership of the tapes, transcripts, and audio/video recordings from the interview(s) they conduct with me and understand that tapes and transcripts will be kept on a password protected Google Team Drive in their possession.

• I understand that information or quotations from the interviews will be published and used in public presentations. I understand I will not receive compensation for the interview.
• I understand that the interview is voluntary and there are no consequences if I choose not to participate. I also understand that I do not have to answer any questions and can end the interview at any time with no consequences.

This research project has been approved on January 25, 2018, by the Institutional Review Board (IRB) at Appalachian State University. This approval will expire on January 24, 2019, unless the IRB renews the approval of this research.

I agree that material from my interview may be used in presentations, or publications resulting from this interview.

By proceeding with the activities described above, I acknowledge that I have read and understand the research procedures outlined in this consent form, and voluntarily agree to participate in this research. VERBAL CONSENT WILL INITIATE THE INTERVIEW ON ZOOM.
