If you are using your driver's license/state ID, you may use it either as a Photo ID or an Address ID but not for both purposes. If you wish to use it as a photo ID, you will need to provide a second ID/Document that shows your address matching section 4 of your USPS form. If you use your license/state ID as an address ID you need to submit another photo ID.
Also please note that a state ID and a Driver's license are considered as the same form of ID and you can only use one of them at a time.
USPS only accepts US-issued photo IDs with the exception of a passport. If you do not have a photo ID issued by the US Government, you will have to use your Passport as your Photo ID.
