MIDDLE SCHOOL SPORTS REGISTRATION INSTRUCTIONS

1. Review the yearly concussion information sheet below

2. Review the yearly sudden cardiac arrest information sheet below

3. Sign-up on the google tryout & information registration form -  BETHANY HERE       ORANGE HERE

4. Complete Form 1 (below) - Please TYPE all information in the fillable PDF - Form must be HAND-SIGNED

5. Complete Form 2 (below) - Please TYPE all information in the fillable PDF - Hand-signed

6. Make sure your child's physical meets the Amity Regional School District's athletic physical requirements (BOE Policy p5141(e))

   1. Fall Sports - physical dated after November 15, 2022

   2. Winter Sports - physical dated after February 22, 2023

   3. Spring Sports - physical dated after May 17, 2023

**Amity Regional School District #5 policy: A student's physical is valid for 13 months AND must also be valid from the first day of tryouts THROUGH the end of the season. It cannot expire at any point during the season per Amity BOE policy 5141(e) . If it does not meet this criteria, you must have a yearly physical or sports physical and submit the form to the nurse. 

7. Print Form (after typing) 1 and 2

8. Sign and Date Forms

9. Submit to the middle school nurse for approval of Physical/Medical Information

10. REGISTRATION WILL BE HELD/REJECTED IF:

    1. Invalid physical (Per BOE policy p5141(e)

    2. Medication Form (new form every new school year)

    3. Outstanding, Unpaid lunch balance
