Transportation Release Form

TEAM TRAVEL & TRAVEL RELEASE FORM

Taken from Page 7 of the Student-Athlete Handbook

All athletes must travel to and from athletic contests in transportation provided by the Athletic Department.

1. Athletes will remain with their team and under the supervision of the coach.

2. Athletes who miss the bus will not be allowed to participate in any contest unless there are extenuating circumstances and with the coach’s approval.

3. Parents will not transport athletes unless a travel release form is completed , (email is unacceptable) are submitted to the Coach preferably 48, but at least 24 hours prior to the contest for approval. The athletic director will give final approval.

4. In an emergency or extenuating circumstances, athletes may travel will an alternative adult. In this rare instance, an alternative adult transportation form must be completed and approved by the athletic director.

5. When transportation is not provided to off-campus home venues, athletes may not transport other student-athletes to any contests or practice sessions.

BANQUETS

Banquet Guidelines:

Individual sport banquets are a way for a team to gather and celebrate a team or program's season. Banquets should not be mini "proms" and should be reasonable in cost. The cost should be reasonable to ensure all can attend.

There is to be NO alcohol served at ANY high school event, including banquets on or off campus

Banquets must be planned as to not interfere with students who participate in other sports. Sunday afternoons are almost always a great time as there is almost never high school sports at this time.

SENIOR GIFTS

Senior Gift Guidelines:

Senior gifts are meant to be momentos of an athlete's participation on a given team. They are not mandatory, but can make for great long-term remembrances of their experience. They should not be gifts such as jewelry, headphones, gift certificates, or any other gift which does not carry long-term meaning and/or memories of their experience. The price should be within reason, $50.00 is a estimation.

50/50 RAFFLES

50/50 Raffle: Required Permit 15 days prior to raffle - Apply through Woodbridge Police Department - Each group allowed 2 per year.
