ENROLLMENT PROCEDURE

Step 1 Fill-out online application form

Step 2 Submit admission requirements

Step 3 Interview by school administrators

Step 4 Payment of Enrolment Fee

Step 5 School ID picture taking

Step 6 Secure admit pass from the Registrar's Office 

ADMISSION REQUIREMENTS

1. Applicants must be at least 16 years old

2. Fill out the LSGH Online Application Form:

3. Submit hard copies of the following documents to the AltEd Registrar's Office:

a. PSA-issued Birth Certificate (photocopy)

b. Latest income tax return of parents or certificate of tax exemption (photocopy)

*special arrangement of the enrolment fee based on the latest ITR submitted

c. Form 137 or ALS Completion Certificate (original copy)

d. Form 138 or ALS Completion Certificate (original copy)

e. Certificate of Good Moral Character (original copy)

f. For Gr. 10 applicants: ESC Certificate (original copy, if applicable)

g. For Gr. 11 applicants: SHS Voucher Certificate (original copy,

if applicable)

h. Medical Certificate (to be submitted after acceptance to LSGH)
