In order to participate in a sport, 3 ENROLLMENT STEPS must be completed by the DUE DATES.
1. REGISTER – click here
2. SUBMIT MEDICAL FORMS – email to: athletics@acs-nj.org
Physical Exam Forms -- new Physical Exam Forms must be submitted if expired or if 1st time
Health History Update Questionnaire -- submit prior to each season
Sports Physical Deferment Form – submit if your Physical will expire during the season
o It is ideal if you can have your Physical Exams completed & submitted in the summer.
o An option is Urgent Care. They can provide a Sports Physical if you cannot get a doctor appointment.
3. PAY FEE(S)
ONLINE (Credit or Debit) -- click here (PREFERRED)
CHECK (no cash) -- ATTN: Athletics
Clearly indicate: Student(s) name & Sport(s).
Submit check(s) to Main Office ONLY
