Sports Forms

In order to be cleared to participate in a sport, you will need to complete the following 3 Steps (also see ACS website).

All 3 steps must be completed and a cleared “Eligibility” Status must be obtained by DUE DATES. (See tracker below for status updates.)

1. REGISTER – click here

--Registration MUST be completed PRIOR to DUE DATE of your 1st sport season.

*This helps with team planning. 

--Please register EACH student for ALL the sports they will be participating in this school year now.

  *This saves you time by registering for all sports up front.

*If you are not sure yet of additional sports, you can register later by that season's due date.

If you registered for a sport and changed your mind, please email: athletics@acs-nj.org.

2. SUBMIT MEDICAL FORMS

--Forms MUST submitted PRIOR to DUE DATE of your 1st sport season

--Please review your student's Physical Exam Expiration Date if previously submitted.

*If you do not know your physical exam expiration date, please email athletics@acs-nj.org.

--If your previously submitted physical exam is valid, it will be noted in the tracker below.

*New Physical Exam Forms must be submitted if expired or if 1st time submitting them.

  *It is ideal if you can have your Physical Exams completed & submitted in the summer by Aug 15.

*An option is Urgent Care. They can provide a Sports Physical if you cannot get a doctor appt.

--3 Ways to Submit the completed Sports Forms:

*UPLOAD HERE (preferred)

*Scan & email to athletics@acs-nj.org

*Hand into Main Office -- in clearly marked envelope

3. PAY FEE(S)

--Payments MUST be made PRIOR to DUE DATE of each season

*If you registered for sports for all seasons now, you may pay for each season separately but payment MUST be received PRIOR to the DUE DATE of each season
