Fill the following sections
· Technology Survey
· Enrollment Form
· Media/Publications Consent & Release for Child/Student
· NY Transportation Information Form
· Request and Authorization Form For Release Of Cumulative Records
· NY Home Language Survey
You can upload any documents, including your child's birth certificate, physical, proof of address and picture.
