Annual Enrollment Fee

I will be charged a $35 enrollment fee per child OR $50 enrollment fee per family upon enrollment, and annually on June 1 of each year. 

 Payment Information

Tuition is based on an average of 4 lessons per month. Some months are longer, others shorter. Holidays and school closings have been taken into account. Tuition is not pro-rated for any reason. Missed classes may not be credited to any other month's tuition and refunds will not be given. By checking this box I have either PAID THE SESSION IN FULL OR AM PAYING MONTHLY. If I am paying TUITION USING my CHECKING/SAVINGS/CREDIT CARD ACCOUNT I Authorize Dance Formations, LLC (DBA Academy of Dance) to initiate debit to my account. Monthly tuition will be drafted on the first of the month, each month, until the session is over or my notice period is complete. I understand refunds are not given for any reason. I also understand my account will automatically be drafted an additional $25 if my auto debit is returned for Non-Sufficient Funds or is returned for any other reason. I will still be responsible for amounts due even if I stop payment on a transaction or have a closed account. In these events I agree to pay an additional $25 collection fee per occurrence and any other legal fees necessary.

 Convenience Fee Credit/Debit Cards

If I use my debit or credit card for payment, I will be charged $5 per transaction convenience fee. If I use my bank account (submitting my routing and account number) I will not be charged any additional fees.

Fundraisers

Although optional, many families prefer to participate and generally make enough from the fundraiser to offset some fees. Any additional money made will go toward tuition/dancewear and other dance related expenses the director sees appropriate. If my family chooses to participate, I understand all funds will become the property of Dance Formations, LLC and will not be given to back to me in the event I withdraw from the studio. All money, including money from fundraisers, paid to Dance Formations, LLC is NON-REFUNDABLE.

 Showcase Fee

There is a $165 SHOWCASE FEE due per family. This fee includes ONE costume per family, UNLIMITED tickets (general admission) to each show, a digital download, 2 recital programs per student, invitations, poster, and participation trophy. This fee is split into 2 payments. The first is due in October and the second payment is due in March. If my family has more than one costume, those fees are due in February. Costumes are $75 per student. THIS FEE EXCLUDES BELLY DANCE CLASS and LYRA CLASSES, POSSIBLY ADULT TAP AS WELL.

 Christmas Celebration Fee

I understand I will be charged a $25 Fee in November for participation in the Christmas Celebration.

Missed Classes

Academy of Dance does not typically offer make-up lessons for missed classes. Each student deserves 100% attention for the class he/she is enrolled in. It is unfair for students in any particular class to be consistently interrupted by others coming in to make-up a lesson. In cases of extreme illness or family emergency Academy of Dance will make an exception. Make up classes may be made in same age group or younger and arrangements must be made within 30 days of absence.

 Inclement Weather Policy

Academy of Dance follows Springdale Public School's inclement weather policy. If I am unsure about class cancellations, I can call the studio, check the facebook page, text or e-mail. In extreme cases the studio may close during evening classes. Classes will not be made up for inclement weather unless the same class has missed two or more classes due to inclement weather.

 Picture and Video Release

Any pictures or video of students may be used for publicity and marketing purposes. However, students names will not be listed. If I do not agree with this I will notify the studio director.

Holiday Closings

Please check to school calendar as this can vary from season to season. A general rule of thumb is to follow Springdale Public School calendar. However, the studio does not necessarily close on every public school holiday.

Withdrawal Period

My student(s) is continually enrolled at Academy of Dance until withdrawal notice is given. At any time I wish to withdraw from classes, I understand a 30 day notice must be given prior to the first of the following month, and the notice must occur within the first 7 days of the monthly withdrawal period. Notice over the phone or by e-mail will not be accepted. To withdraw, I must come into the studio office and fill out a withdrawal form in person. Although a 30 day notice is given, any remaining days of the month paid past the 30 day notice are not refunded. For instance, if I withdraw after the first 7 days of the required 30 day notice period, I will still owe tuition for the month of the notice period and the entire following month.

 Assumption of Risk

Dance Formations, LLC, and its owners and contractors, will not be held liable for any accidents or injuries which cause bodily harm or death to students while they are under the supervision of any employee or contractor of Dance Formations, LLC. Students are to wait inside the building to be picked up after class, and the studio owner, staff and contractors will not be responsible and/or held liable after the student leaves the classroom. This release information is also effective for activities which students participate that are not held at the studio location (recital, exhibitions,etc.) In case of injury or death, Dance Formations, LLC and it owners and contractors will not be responsible for any and all fault and/or expenses related to the incident.

 Assumption of Risk and Waiver of Liability (Covid)

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of large groups of people. Academy of Dance ("the Studio") has put in place preventative measures to reduce the spread of COVID-19; however, the Studio cannot guarantee that you or your legal dependents ("your participants") will not become infected with COVID-19. Further, attending and participating in Studio activities could increase the risk of you and your participants of contracting COVID-19. By signing this document, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you and your participants may be exposed to or infected by COVID-19 by attending Studio activities and that such exposure or infection may result in personal injury, illness, permanent disability, and death. You understand that the risk of becoming exposed to or infected by COVID-19 at Studio activities may result from the actions, omissions, or negligence of Academy of Dance and others, including, but not limited to, Studio employees, Studio contractors, members, other participants, and their families. You voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to you or your participants (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that you or your participants may experience or incur in connection with you or your participants' attendance at the Studio or participation in Studio activities ("Claims"). On your behalf, and on behalf of your participants, you hereby release, covenant not to sue, discharge, and hold harmless the Studio, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs, or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Studio, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Studio activity. You further acknowledge and agree that the Studio has no duty to inform you when Studio employees, Studio contractors, members, other participants, or their families test positive for COVID-19, and you understand that as a matter of state and federal privacy laws, the Studio cannot disclose personal health information to the public.
