Attention: Rising 12th Grade Students
Rising 12th Student Requirement:
Booster/Second Meningococcal Conjugate Vaccine
All 12 graders are required to have a second dose, also referred to as a booster dose, of meningococcal conjugate vaccine. Please contact your health care provider or Alamance County Health Department for verification that your child has received this vaccine & schedule an appointment if not. You will have 30 calendar days from the first school day to provide the required up-to-date immunization record. If proof of vaccination is not provided by the 30th calendar day, your child will be excluded from school until proof of immunization is received.

New Students
All newly enrolled students must have proof of receiving their required immunizations. If this is their first time entry into North Carolina public schools, then it is required to provide proof of immunizations and turn in a completed North Carolina Health Assessment within 30 days of their enrollment date. The health assessment must be completed by a medical provider and/or the local health department within one year (365 days) prior to the first day of attendance.

North Carolina State immunization and health assessment laws require these items are turned into the school by the 30th day. Students are not permitted to return to school after the 30th calendar day without these items. Please contact your school nurse for any questions &/or concerns.
